Who Qualifies

How to Apply

• Gather discharge papers (DD-214) and current medical records
• Document monthly income and out-of-pocket medical and care costs
• Submit VA Form 21-2680, signed by a physician
• Work with an accredited veteran service officer (VSO) — free of charge
