Medicaid / BadgerCare Plus

Who Qualifies

Wisconsin residents with limited income and assets. Income and asset limits vary by program category.

What It Covers

Long-term care in nursing homes, some in-home care, and limited assisted living support.

How to Apply

Apply through Wisconsin DHS at access.wi.gov or call your local ADRC.

Tip: Medicaid is the primary payer for long-term care in Wisconsin. Rules are complex — consult an elder law attorney before applying.

Family Care / Family Care Partnership

Who Qualifies

Wisconsin seniors who meet functional and financial eligibility for long-term care. Available in all Wisconsin counties.

What It Covers

Personal care, home modifications, adult day care, respite care, home-delivered meals, and assisted living support.

How to Apply

Contact your local Aging and Disability Resource Center (ADRC) for a functional screen and enrollment.

Tip: Family Care is Wisconsin's managed long-term care program. It can pay for services that help seniors stay at home longer.

IRIS (Include, Respect, I Self-Direct)

Who Qualifies

Wisconsin seniors eligible for long-term care who want to self-direct their services and hire their own caregivers.

What It Covers

Self-directed personal care, home modifications, assistive technology, and the ability to hire family members as caregivers in some cases.

How to Apply

Contact your local ADRC for screening. IRIS Consultants help you develop a personalized care plan and budget.

Tip: IRIS gives you control over who provides your care. You manage a budget and can hire workers you choose — including some family members.

VA Aid & Attendance Pension

Who Qualifies

Wartime veterans or surviving spouses with limited income. Veteran must have served 90+ days with at least 1 day during wartime.

What It Covers

Monthly cash benefit up to ~$2,300/month (veteran) or ~$1,500/month (surviving spouse) that can be used for assisted living, in-home care, or nursing home costs.

How to Apply

Apply through VA Form 21-2680. Contact your County Veterans Service Officer (CVSO) for free help.

Tip: This is one of the most underutilized benefits. Many Wisconsin veterans qualify but never apply. The benefit is tax-free.

Medicare Savings Programs

Who Qualifies

Seniors on Medicare with limited income and resources. Four levels: QMB, SLMB, QI, and QDWI.

What It Covers

Help paying Medicare Part B premiums, deductibles, and co-insurance. QMB covers all Medicare cost-sharing.

How to Apply

Apply through Wisconsin DHS or your local ADRC. Reapply annually — eligibility can change.

Tip: People often confuse Medicare with Medicaid. Medicare does NOT pay for long-term custodial care. These savings programs help reduce Medicare costs specifically.

Supplemental Security Income (SSI)

Who Qualifies

Seniors 65+ with very limited income and resources. Federal program administered by Social Security.

What It Covers

Monthly cash payment to help with basic needs — food, clothing, shelter. Amount varies by living situation.

How to Apply

Apply online at ssa.gov/ssi or call 1-800-772-1213. In-person appointments available at local Social Security offices.

Tip: SSI eligibility often opens the door to Medicaid eligibility in Wisconsin. If you qualify for SSI, you typically qualify for Medicaid.
